Various regulations (state, federal) require us to share certain information with you. Please feel free to let your provider know if you have questions about any of this information.
Filing A Complaint
If you have concerns about our sessions, please speak with your provider so they can address them. Comments will be handled respectfully. You may request a referral to another provider or end our relationship at any time. You are entitled to considerate, safe, and respectful care, regardless of minority status or payment source. You may ask questions about any aspect of our relationship or your provider’s qualifications. Your provider will not attempt to form social or sexual relationships with clients or former clients. If you believe that we have not responded appropriately, or have acted unethically, you can file a formal complaint with the licensing authority in your state:

Request a Copy of Your Records
Please request during session or send a message to admin@finselfcounseling.com or call/text (972) 895-8376. We recommend discussing with your clinician in advance. Please be specific about the need for records (what they will be used for). It is our responsibility to protect your confidential information, as we cannot control what happens to it when it leaves our office. Whenever possible, a clinical summary is preferable to a full records release. If it involves a legal issue, we may request to discuss with your attorney to clarify what specifically they need, so as not to release more information than necessary. You may be asked to complete a Release of Information and an acknowledgement of understanding about the risks of releasing confidential information.
For Uninsured And/Or Private Pay Clients: No Surprises Act Statement ("Good Faith Estimate")
The No Surprises Act requires healthcare providers to provide clients with a “good faith estimate” of the costs relating to their care. Costs for services are listed on our Fee Information form which you will receive prior to starting services. We will always notify you 30-60 days in advance should any costs for psychotherapy services change. The cost of your services will vary based on (but not limited to): how many sessions you complete; how many sessions you cancel/cancel late/no-show; what kinds of services you receive; whether it is a crisis (i.e. hospital visit); what your insurance plan might or might not cover for an out-of-network provider; what state you live in. If you submit your own claims to your insurance plan, we cannot guarantee that your plan will cover some or all of your costs. Please feel free to discuss further with your provider if you have any questions.
